The Burden of Skin Disorders in India
India's tropical climate, high population density, and diverse environmental exposures create a uniquely high prevalence of dermatological conditions. An estimated 10–15% of all outpatient visits in India involve skin complaints, making dermatology one of the busiest outpatient specialties.
Fungal skin infections (dermatophytosis) are among the most prevalent skin conditions, affecting hundreds of millions of Indians. Inflammatory skin diseases — eczema, psoriasis, acne — cause significant morbidity, while infectious conditions like scabies and leprosy continue to represent public health challenges in certain regions.
Antifungal Dermatology: India's Biggest Skin Challenge
Dermatophytosis (Ringworm / Tinea)
Tinea corporis, tinea cruris, tinea pedis, and onychomycosis have reached near-epidemic proportions in India, particularly over the past decade. Contributing factors include the widespread misuse of topical corticosteroid-antifungal combinations (especially FDC creams containing betamethasone, clotrimazole, and neomycin), which suppress inflammation without adequately treating the fungal infection — creating a chronic, recurrent, and increasingly resistant infection pattern.
Terbinafine (topical and oral) is the most effective treatment for dermatophytosis. Itraconazole is used for onychomycosis (nail fungal infection) requiring pulse therapy. Fluconazole is less effective against dermatophytes. Topical agents alone are insufficient for extensive or nail infections.
Topical Corticosteroids: Potency, Selection, and Safety
Topical corticosteroids are the most prescribed topical medicines in dermatology, used across inflammatory skin conditions including eczema (atopic dermatitis), psoriasis, contact dermatitis, and seborrhoeic dermatitis. They are classified by potency:
- Mild: Hydrocortisone 1% — safe for face, flexures, and children
- Moderate: Clobetasone butyrate 0.05%, Triamcinolone — for body eczema
- Potent: Betamethasone valerate 0.1%, Mometasone furoate — for thick plaques
- Very potent: Clobetasol propionate 0.05% — for severe psoriasis, palmoplantar eczema (short-term only)
Potent and very potent corticosteroids should never be used on the face, groin, or in children without specialist supervision. Long-term use causes skin atrophy, striae, telangiectasia, and systemic absorption effects.
Acne Vulgaris: A Major Quality-of-Life Condition
Acne affects an estimated 85% of adolescents and a significant proportion of adults in India. Effective pharmacological management depends on severity:
- Mild acne: Topical Benzoyl Peroxide, Adapalene (retinoid), Clindamycin (antibiotic)
- Moderate acne: Combination topical therapy plus oral antibiotics (Doxycycline, Minocycline)
- Severe/nodular acne: Oral Isotretinoin — the only treatment targeting all four pathogenic factors of acne, with teratogenic risk requiring strict pregnancy prevention measures in women
Psoriasis: A Systemic Inflammatory Condition
Psoriasis is a chronic immune-mediated condition affecting 2–3% of Indians. Management ranges from topical therapy for limited disease to systemic treatments for moderate-to-severe disease:
- Topical: Corticosteroids, Vitamin D analogues (Calcipotriol), Coal tar preparations, Salicylic acid (keratolytic)
- Systemic: Methotrexate, Ciclosporin, Acitretin for moderate-severe disease
- Biologics: TNF-alpha inhibitors and IL-17 inhibitors for refractory disease
Eczema (Atopic Dermatitis) Management
Atopic dermatitis is the most common inflammatory skin condition in Indian children. Management centres on skin barrier repair (emollients, moisturisers), trigger avoidance, and topical anti-inflammatory therapy. Topical calcineurin inhibitors (Tacrolimus, Pimecrolimus) offer steroid-sparing alternatives for sensitive skin areas.
Scabies and Other Ectoparasitic Infections
Scabies remains endemic in overcrowded communities across India. Permethrin 5% cream is the first-line topical treatment, applied overnight to the entire body surface. Oral Ivermectin is increasingly used for widespread outbreaks and in institutional settings. Crusted (Norwegian) scabies in immunocompromised patients requires combination therapy.
