GASTRO & NUTRITION · KASDAP HEALTHCARE
Gastrointestinal disorders and nutritional deficiencies together represent one of the largest therapeutic segments in Indian healthcare — affecting hundreds of millions of patients across all age groups.
Gastrointestinal Health in India: An Enormous Clinical Need
Gastrointestinal (GI) disorders are among the most prevalent health conditions in India, spanning acute infections, chronic functional disorders, inflammatory conditions, and nutritional deficiencies. From the rural patient with acute gastroenteritis to the urban professional managing GERD from dietary habits and stress, the spectrum of gastro and nutritional health needs in India is vast and clinically significant.
India's gastro pharmaceutical market is estimated at over ₹12,000 crore and growing consistently, driven by increasing urbanisation, changing dietary patterns, rising stress levels, and improved diagnostic rates for conditions like H. pylori infection, inflammatory bowel disease, and non-alcoholic fatty liver disease.
Proton Pump Inhibitors: The Most Prescribed GI Medicines
Proton Pump Inhibitors (PPIs) are consistently among the top prescribed drug classes in India. By irreversibly inhibiting the H+/K+-ATPase enzyme in gastric parietal cells, they produce profound and sustained suppression of gastric acid secretion.
Clinical Applications of PPIs
- GERD: First-line treatment for gastroesophageal reflux disease and erosive esophagitis
- Peptic Ulcer Disease: Used in combination with antibiotics for H. pylori eradication therapy
- NSAID-Induced Gastropathy: Co-prescribed with NSAIDs to protect the gastric mucosa in patients on long-term anti-inflammatory therapy
- Zollinger-Ellison Syndrome: High-dose PPIs are the treatment of choice for this rare condition of excessive acid secretion
H. pylori Eradication: A Key Clinical Priority
Helicobacter pylori infection affects an estimated 50–60% of Indians, contributing significantly to peptic ulcer disease, chronic gastritis, and gastric cancer risk. Standard triple therapy consists of a PPI plus two antibiotics (typically Clarithromycin and Amoxicillin or Metronidazole) for 14 days. Bismuth-containing quadruple therapy is recommended in areas of high clarithromycin resistance.
Antispasmodics and Prokinetics
Antispasmodics
Dicyclomine, Hyoscine (Buscopan), and Mebeverine are used to manage smooth muscle spasm in conditions including IBS, biliary colic, and dysmenorrhea. They provide symptomatic relief by relaxing intestinal smooth muscle without affecting gut motility.
Prokinetics
Domperidone, Metoclopramide, and Mosapride improve gastric emptying and small intestinal motility, and are used for gastroparesis, GERD, nausea, and vomiting. Itopride has emerged as a well-tolerated prokinetic with fewer central nervous system side effects than Metoclopramide.
Laxatives and Antidiarrheals
Laxatives: A Broad Category
- Osmotic laxatives: Lactulose and Polyethylene Glycol (PEG) are safe for long-term use and first-line for chronic constipation
- Stimulant laxatives: Bisacodyl and Senna for acute constipation relief
- Bulk-forming agents: Ispaghula (Psyllium husk) adds dietary fibre and improves stool consistency
- Stool softeners: Docusate sodium for preventing straining in post-operative and cardiac patients
Antidiarrheals
Loperamide remains the most effective over-the-counter antidiarrheal for acute non-infectious diarrhoea. Racecadotril (Enkephalinase inhibitor) is increasingly preferred, particularly in paediatric diarrhoea, as it reduces intestinal secretion without affecting motility. Oral Rehydration Salts (ORS) remain the cornerstone of diarrhoea management across all age groups.
Nutritional Deficiencies: India's Hidden Health Crisis
Iron Deficiency Anemia
Iron deficiency anemia is the most prevalent nutritional deficiency in India, affecting over 50% of women of reproductive age, 60% of pregnant women, and a significant proportion of children under five. Treatment requires oral iron supplementation — typically Ferrous Sulphate, Ferrous Fumarate, or newer iron formulations like Iron Polymaltose Complex (IPC) which offer better GI tolerability.
Vitamin D Deficiency
Despite abundant sunshine, Vitamin D deficiency affects an estimated 70–80% of Indians. Contributing factors include indoor lifestyles, sunscreen use, air pollution, and dietary habits. Oral Vitamin D3 (Cholecalciferol) supplementation is indicated for correction and maintenance, with weekly high-dose therapy for rapid correction of severe deficiency.
Vitamin B12 Deficiency
B12 deficiency is widespread in India, particularly among vegetarians who represent a significant proportion of the population. It presents with megaloblastic anemia, peripheral neuropathy, and cognitive decline. Methylcobalamin is the preferred form for neurological manifestations, while Cyanocobalamin offers a cost-effective alternative for hematological correction.
Calcium and Bone Health
Calcium carbonate and Calcium citrate, combined with Vitamin D3, form the foundation of osteoporosis prevention and management. Bisphosphonates (Alendronate, Risedronate) are added for established osteoporosis in postmenopausal women and elderly patients.
